Unless things have changed recently, it's more of a monopoly transit provider than an exchange point. It's a service of Emirates Telecom/ Etisalat, AS8966; it's who people are obliged to buy transit from, there being no alternative for licensed ISPs. They do like to use the word "exchange" though, which can give the wrong impression.

If it seems like a closed club, perhaps that's more of an ISP licensing issue in UAE than anything else.
